Payment at time of service:

First Office Call: 1 1/2 hours - $120
Return Office Call: 1 hour - $80

I accept Aetna, Blue Cross/Blue Shield, Federal Employee Program, First Choice Health Network, Lifewise, Personal Injury Protection/Auto Injury, Premera, Regence, Providence Health Plan, Uniform/Regence and United Health insurance plans; for all other insurance plans I can provide a super bill for you to send into your insurance company that may result in a full to partial compensation. The above fees are payment at time of service
